What is a virtual ward?

A virtual hospital mirrors that of a physical hospital; it has an emergency department (Urgent Community Response teams (UCRT)), and wards (the frailty virtual ward and respiratory virtual ward), into which a patient can be transferred. It is a model of care for people presenting with urgent and complex health and care needs to be supported, monitored and treated in their own homes.

For some patients, a virtual ward provides a safe alternative to hospital that benefits both the patient and the NHS providers.

The Mid and South Essex Virtual Hospitals programme is a service that works across the Mid and South Essex Community Collaborative (MSECC) – a partnership.

Through our virtual hospital’s initiatives our frailty and respiratory virtual ward teams can respond to the needs of patients in their own homes, including care homes. They provide hospital-level care and remote monitoring for people living with frailty or with an acute respiratory infection who would otherwise be in hospital. This prevents admissions or allows patients to return home sooner to continue their treatment at home. It means care is more personalised, delivering it to the patient at the right place and the right time.

Frailty virtual ward

Our frailty virtual ward is for patients with frailty or complex comorbidities, who may have an urgent care need or an underlying long term condition.

Working as part of our frailty ward, you will be caring for patients who may have been in hospital or have been transferred by the Urgent Care Response Team (who support patients in their own homes, including nursing and residential care homes) to avoid admission to hospital.

Patients may stay in the virtual wards for up to a period of 14 days.

Respiratory virtual ward

The respiratory virtual ward works a little differently to frailty. It is an additional service that sits within the local respiratory teams. It is for patients who primarily present with a condition that is an exacerbation of an underlying respiratory disease which has previously been diagnosed.

Working as part of our respiratory virtual ward, you will provide community care for patients that meet criteria closer to home, avoiding hospital admission. The respiratory virtual ward is a 24/7 service and each area has a 14 bed capacity.

Due to the nature of an integrated team, you will work within the respiration virtual ward and also support all other deliverable aspects of the respiratory teams in the community.

The Integrated Respiratory team encompasses Chronic Obstructive Pulmonary Disease (COPD) team, Pulmonary Rehabilitation, Home Oxygen service, breathlessness and diagnostics (spirometry) and the respiratory virtual ward.

You will have opportunities to work across all aspects of the service.
